These materials are licensed to one practice at one physical location. Print them, mark them up, and circulate them to the people who work there. Please do not share, forward, or post them outside that location. Additional locations need their own license, and the terms of sale state the scope plainly.

If something is wrong with a download

Email [email protected] and we will fix it. A failed download or a damaged file is a delivery problem, and we will not make you argue about it.

New editions. We publish a new edition whenever something material changes, and we email past buyers when we do. If you bought within the 90 days before a new edition is released, it is yours at no cost. After that, it is offered to you at a discount.
